Position Summary:
The Clinical Pharmacist is responsible for providing patient care through the provision of direct operational and/or patient care services. The Clinical Pharmacist reports directly to the Director of Pharmacy. The Clinical Pharmacist is responsible for ensuring safe and appropriate use of medications through the provision of drug information, drug therapy monitoring, distributive and supervisory pharmacy services within the framework of company policy, standard practices, and state/federal laws. The Clinical Pharmacist provides education to the interdisciplinary team (physicians, mid-level providers, nursing), patients and caregivers, the pharmacy department, as well as pharmacy students and residents. Other responsibilities include providing direct patient care guided by protocols and collaborative practice agreements, active participation in the pharmacy residency program, serving on committees within Tandem Health, and expanding clinical pharmacy services as opportunities arise.
