The Community Care Aide positioncombines the regular responsibilities of a Home Health Aide with additionalduties focused on engaging with residents and their loved ones, identifyingpotential residents in need of care, and serving as a liaison between theagency and the assisted living facility.The Community Care Aide will work within an assisted livingfacility, bridging the gap to create and expand conversations concerningpatient care, communication, and follow-up.This role is ideal for someone who has experience as an HHA and wants toapply their knowledge and passion for the field to create a better healthcaremechanism for patients and their families.

Key Responsibilities:

• Provide hands-onassistance with activities of daily living (ADLs) as required by residents.
• Assist with grooming,bathing, dressing, ambulation, and other personal care tasks.
• Engage with residents andtheir loved ones in a friendly and professional manner.
• Initiate and participatein conversations to create a positive and supportive environment.
• Facilitate andparticipate in activities to enhance residents' quality of life.
• Identify potentialresidents in need of home care services and communicate this information to theagency promptly.
• Assist residents withactivities such as carrying & unpacking groceries, provide directionswithin the community, and support with daily tasks.
• Establish and maintainpositive relationships with community staff.
• Collaborate effectively& seamlessly with the assisted living facility's care team, presenting asan extension of their services.
• Be observant andattentive to residents' needs, noting any changes in their condition or needfor additional care.
• Communicate effectivelywith agency and community staff to ensure a smooth flow of information.
• Maintain a cheerful,professional demeanor at all times.
• Present well-groomed andappropriately dressed in accordance with agency and community standards.

Qualifications:

• Must possess a valid andcurrent New York State HHA certification.
• Minimum of 5 years ofexperience as a Home Health Aide preferred.
• Ability to add andsubtract two-digit numbers and to multiply and divide by 10 and 100. Ability toperform these operations using units of American money, American weightmeasurement, volume, and distance.
• Ability to apply commonsense understanding to carry out simple one or two-step instructions.
• Excellent communicationand interpersonal skills.
• Ability to buildrelationships with residents, their families, and community staff.
• Ability to speak, read,and write in English sufficiently to understand and interpret the HHA Plan ofCare, document care provided on the HHA Time and Activity Report, and able tocall the agency to report changes and/or issues related to the patient and/or911 in case of an emergency.
• Proven self-starter withthe ability to work independently.
• Demonstrated ability toidentify and address residents' needs proactively.
• Strong observationalskills with attention to detail.
