Summary
The Certified Home Health Aide is a member of the Hospice Interdisciplinary Team and works under the supervision of an R.N. The position is responsible for providing personal care and light homemaker services to terminally ill patients of all ages in their homes in order to provide comfort to the patient, their family and/or care givers. Position also assists with maintaining terminally ill patients in the home and promoting overall quality of life. This position also provides office support duties as census allows.
Essential Duties and Responsibilities
  • Provides personal care and light homemaker services to patients in their homes in order to increase comfort and quality of life, such as bathing, assisting with patient ambulation, preparation of meals and special diets, and performing light household services.
  • Assists families and care givers in provision of personal and other appropriate care of patient in order to increase their involvement and confidence in care of their loved one.
  • Specific skills within scope of practice include:
    • Communication skills, including the ability to read, write, and verbally report clinical information to patients, representatives, and caregivers, as well as to other Home Health Agency (HHA) staff.
    • Observation, reporting, and documentation of patient status and the care or service furnished.
    • Reading and recording temperature, pulse, and respiration.
    • Basic infection prevention and control procedures.
    • Basic elements of body functioning and changes in body function that must be reported to an aide's supervisor.
    • Maintenance of a clean, safe, and healthy environment.
    • Recognizing emergencies and the knowledge of instituting emergency procedures and their application.
    • The physical, emotional, and developmental needs of and ways to work with the populations served by the HHA, including the need for respect for the patient, his or her privacy, and his or her property.
    • Appropriate and safe techniques in performing personal hygiene and grooming tasks that include:
      • Bed bath; Sponge, tub, and shower bath; Hair shampooing in sink, tub, and bed; Nail and skin care; Oral hygiene; Toileting and elimination; Safe transfer techniques and ambulation; Normal range of motion and positioning; Adequate nutrition and fluid intake; Recognizing and reporting changes in skin condition.
  • Assists in the provision of therapy services supervised by the appropriate therapists in the home to increase utilization of therapy.
  • Documents activity and care on required forms/reports to assure communication and consistency and meet regulatory requirements.
  • Participates in interdisciplinary team activities and decision making as a team member to provide consistent and appropriate care planning.
  • Reports significant changes in patient condition noted while providing patient care to appropriate personnel to assure proper interventions and actions are taken.
  • Participates in the Hospice Quality Assurance and Performance Improvement Program.
  • Performs other related duties as required.
  • Light housekeeping activities include, but are not limited to: light dusting, sweeping, vacuuming, mopping floors, storing equipment and foods properly, cleaning bathrooms with disinfectant, changing linens and laundering patient clothing.
  • Provides Home Health and Hospice office support.
  • Other duties as may be assigned.
  • Demonstrates System Values in performance and behavior.
  • Complies with System policies and procedures.

Education and Experience
No educational requirement.
One to three months related experience and/or training.
Required License and Certificates
California Home Health Aide certificate - Upon hire
Valid Driver's License - Upon hire
Basic Life Support (BLS) certificate for healthcare provider - Upon hire
BLS: Employee will be enrolled in the Resuscitation Quality Improvement (RQI) Basic Life Support (BLS) Entry or Prep Curriculum (depending on their previous BLS certification) - Upon hire
Other Experience/Qualifications
  • Emotional maturity, reliability and ability to cope with problems of the terminally ill in the home.
  • Minimum of one year experience within the last three years as a Certified Home Health Aide.
  • Experience with care of terminally ill patients in the home.
  • Completes educational competencies.
