As a Home Health Nurse at TONE, your primary focus will be on empowering our patients to regain independence and improve their quality of life. Your duties will revolve around:

• Delivering specialized care tailored to the unique needs of each patient, guiding them towards greater independence.
• Conducting comprehensive initial assessments to craft individualized care plans in coordination with physicians and the interdisciplinary team.
• Educating patients and their families on effective care strategies and lifestyle enhancements to promote their well-being.
• Coordinating and administering specialized treatments designed to enhance the health and overall well-being of our patients.

Qualifications

Qualifications:

• Valid registered nursing license (required)
• 2 years of nursing experience
• Associate degree in nursing (required)
• Home health experience is recommended
• Valid BLS (CPR) Certification
