System Administrative Services Corporate is Hiring a Remote Centralized Admissions Coordinator

Other duties may be assigned. Directly oversees all aspects of admission process for all market skilled nursing centers and market admissions associates. Develops and implements training programs and professional development plans to educate Admissions Coordinators, Admissions Assistants and other team members on skilled nursing admission guidelines and processes. Participate in Nursing Center QAPI process. Responsible for meeting established goals related to admissions for market skilled nursing centers, referral source relationship development, outreach program implementation and other quality metrics established. Communicates with organization personnel, especially designated market Patient Transition Directors regarding patient transitions. Develops and coordinates admissions associates in strategic outreach activity in market in order to facilitate referrals to skilled nursing centers. Performs strategic outreach events in market in order to increase community awareness and facilitate referrals to skilled nursing centers. Reviews monthly analytic data. Tracks information to determine market trends, and develops action plans based on data relative to key performance indicators. Attend and participate in continuing educational programs to keep abreast of changes in your field as well as to maintain current license/certification, as required. Attend and participate in mandatory in-services. Honor patient’s rights to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights, including the right to wage complaints. Comply with Corporate Compliance Program. Report job-related functions/tasks that involve occupational hazards including exposure to blood and body fluids and others as necessary.
