We are hiring a Client Care Coordinator who can answer yes to these questions. At SYNERGY HomeCare, we create a world of care at home for all, which means you will have the opportunity to support a dedicated team in caring for a wide variety of people, spanning all ages and all abilities. We are seeking a Client Care Coordinator to join our independently owned and operated national agency.

We are scheduling interviews immediately for the Client Care Coordinator position. What you should know about the Client Care Coordinator position with SYNERGY HomeCare:

Responsibility:

  • Establish and maintain relationships with referral sources, including hospital case managers, assisted living facilities, skilled nursing/rehab facilities, home health agencies, hospices, physicians’ offices, fiduciaries, etc.
  • Complete daily face-to-face call with referral sources for purposes of generating business leads.
  • Plan and organize community marketing events to enhance our visibility in the community and generate new relationships and business opportunities.
  • Support Office staff in managing schedules and client/ caregiver relations.
  • Willing and able to cover shifts for callouts and scheduling needs.
  • Available for weekend/after-hours on call rotation and wok flexible hours.
  • Maintain computer schedules and ensure timely data entry for clients and caregivers.
  • Maintain confidentiality of all information pertaining to employees and clients 
  • Perform other functions as deemed appropriate by the management team.

Skills and Experience:

  • Knowledgeable and experienced in the healthcare and senior care industries,
  • with contacts and relationships that can be leveraged for business
  • development preferred.
  • LVN or RN degree preferred.
  • National Background check
  • Home care and administrative work experience preferred.
  • Strong phone skills and experience in cold calling.
  • Ability to multitask and work well under time pressure.
  • Positive, friendly attitude and ability to work well in a close team environment.
  • Can work flexible hours including weekends.
  • Strong interpersonal, presentation, and communication skills
  • Strong computer skills and knowledge of CRM processes.
  • Scheduling/staffing experience preferred.
