 LPN - Client Care Manager Requirements:

• Conduct initial client assessment
• Plan, develop, and implement Plan of Care
• Make quality visits to client homes
• Evaluate Plan of Care effectiveness, modify as needed
• Provide skills training and coaching to caregivers
• Assist with client education
• Assist with client medication management
• Complete all documentation in accordance with Ohio State laws

Qualifications & Requirements

• Current LPN certification
• Home Health/Home Care experience a plus
• Current CPR certification
• Reliable transportation/valid driver's license
• Must be willing to undergo a national background check and a drug screening
• Positive, friendly attitude and ability to work well in a close, team environment
• Self-motivated and strategic decision maker
