We are hiring a Client Care Coordinator who can answer yes to these questions. We are seeking a Client Care Coordinator to join our independently owned and operated national agency.

We are scheduling interviews immediately for the Client Care Coordinator position. What you should know about the position with SYNERGY HomeCare:

Client Care Coordinator Benefits:

  • Hourly pay starting at $18 and up, paid bi-weekly, with growth potential

Client Care Coordinator Responsibilities:

  • Handling incoming client inquiry calls and finding solutions
  • Completing home assessments / home visits for new clients, and initiating the plan of care
  • Meeting with clients and introducing care professionals
  • Providing exemplary customer service
  • Communicating and engaging regularly with clients, their families and care professionals
  • Performing schedule modifications to existing schedule
  • Assisting with hands-on training for care professionals
  • Reviewing care notes and highlighting areas of interest
  • Be available and provide back-up client care as needed

Client Care Coordinator Requirements:

  • Experience – At least three years medical or home care office administration and scheduling experience. Experience providing personal care. 
  • Education – High school diploma and some college preferred. Medical Assistant (MA) certification and/or CNA preferred
  • Skills – Developing personal relationships, time management skills, exemplary communication skills
  • Schedule - Availability to contribute to the team 40 hours per week Monday through Friday and some weekends
  • Location - Must live in or near Brentwood/Franklin, TN. This is mostly an office position (office in Franklin, TN), requiring presence in the office, local in-home engagement opportunities with care professionals, clients and families
  • Characteristics – Leadership characteristics with a positive, helpful attitude, self-motivated and willing to learn and grow
  • Must be at least 18 years or older
