Looking for a DYNAMIC Social Service Director to join our team!

As the Social Service Director you will be responsible for the planning, organizing and managing of Social Service needs within your facility. Along with the management of your department, you will be responsible for additional duties such as:

> Care Planning, and Discharge Planning for facility residents.

> Initiate family involvement as needed.

> Provide oversight of the social, psychological and emotional well being of the resident. 

> Ensure that the Social Service function of the MDS is completed with accuracy and in a timely fashion.

> Assist resident/s with community service support as needed.

EXPERIENCE: 

Prior SNF experience preferred.
