POSITION OVERVIEW:

Works collaboratively with providers, staff and other health care professionals to provide a medical home and care coordination across the healthcare continuum for all patients within the office setting. Is an integral member of the health care team who strives to promote patient wellness, improve care outcomes, efficient utilization of health services lowering health care costs and maintain high quality standards of care among a patient population with complex health needs. Also responsible for coordinating a wide range of self-management support and disease management activities for the clinic's patient population. Care manager will be cross trained with the Annual Wellness employee. Receives direction from: Providers and Clinical Managers/Supervisors. Gives direction to : Staff RN's, LPN's, CMA's, CNA's and Schedulers. 

EDUCATION:

• Graduate of an accredited Nursing Program

EXPERIENCE:

• Minimum 2 years of nursing experience
• Basic computer and keyboarding skills
• Microsoft Word and Excel preferred. 
• Care Management experience is desirable

LICENSE/CERTIFICATIONS:

• MT Nursing licensure. 
• BLS
• Care Management certification or ability to obtain within 6 months of hire

KNOWLEDGE/SKILLS/ABILITIES:

• Medical Terminology
• Basic Nursing Principles and Procedures. 
• Evidence of essential leadership, communication education and counseling
• Highly organized with ability to keep accurate notes and records
