Full Time – Days

Surgeons Choice Medical Center is a patient centered health care facility and physician owned destination of care focusing on all hand, joint, orthopedic and sports medicine. In 2004, in an ambitious push to bring hospitality back to the hospital, a small group of top Metro-Detroit surgeons decided to create their own surgical hospital; one that provided patients with the best possible care in a small, easy-to-manage environment that truly embraces the best patient experience. We have since become the premier center of choice with 30 surgical beds, 26 rehabilitation beds, and 6 operating rooms.

Surgeons Choice Medical Center has an exciting opportunity for a Licensed Social Worker. An ideal candidate has a passion to serve people to improve their quality of life and empower them on their health journey. We are looking for someone with Skilled Nursing and Rehabilitation experience. This is a great opportunity to work with a small group of patients (26 bed facility).

Responsibilities:

  • Ensures that a quality social work program for the individual patient and family is developed and implemented while concurrently monitoring service utilization and length of stay appropriateness.
  • Conducts evaluations to determine the patient’s discharge plan, discharge needs, emotional and social needs.
  • Encourage and enhance patient, family and other necessary agents in the involvement in the treatment and disposition of planning.
  • Facilitate post-dispositional adjustment of the patient and family.
  • Enhance understanding and adjustment to the patient’s disability by providing support, education and training.
  • Assist with arranging family meetings and post-discharge services.
  • Communicate with the clinical staff as needed for clarification of the patient’s status and update them on the discharge plan.
  • Provides documentation in a legible manner to maintain the quality of the medical record as it relates to reimbursement guidelines, accuracy, and facility requirements.
  • Participates in team and family conferences as necessary.
  • Completes all MDS appropriate documentation and interviews.
  • Promotes current standards of clinical practice through participation in continuing education and in-service programs.
  • Participates in program development.
  • Provides in-services to other staff members on Social Work and patient rights issues as it relates to patient care.
  • Performs all other duties as assigned.

Requirements:

  • Bachelor’s Degree in Social Work or Human Services from an accredited school required.
  • 2 years of Social Work experience required.
  • Previous experience in health care setting strongly preferred.
  • Current unrestricted Social Work License from the State of Michigan required.
  • Ability to react effectively to the needs of patients and families.
  • Must possess excellent written and verbal communication skills.
  • Ability to exercise professional judgment and initiative when analyzing problems and recommending solutions.
  • Knowledge of state and federal regulations required for Social Work departments in Nursing homes.
  • Basic computer skills, including use of Microsoft Office software.

Work Environment:

  • Exposure to patient care areas.
  • May be exposed to blood and body fluids, sharp blunt or irregular surface objects, and hazardous chemicals.
