Job Description: We are seeking a compassionate and skilled Medical Social Worker (MSW) to join our home health care team. As a Medical Social Worker, you will play a vital role in providing comprehensive support to our patients and their families, ensuring their emotional, social, and practical needs are addressed during their home health care journey.

Qualifications
- Bachelor's or Master's degree in Social Work or related field
- Valid state license to practice as a Social Worker
- Minimum of 2 years of experience in home health care
- Proficiency in behavioral therapy, patient care, and medical documentation
- Knowledge of HIPAA regulations and medical terminology
- Experience in conducting patient assessments and developing treatment plans
- Ability to work with diverse populations, including children, individuals with developmental disabilities, and older adults
- Strong communication and interpersonal skills
- Familiarity with EMR systems and medical records management
- Certification in Applied Behavior Analysis or related field is a plus
- Experience in crisis intervention and counseling is highly desirable

Responsibilities:

• Conduct initial assessments of patients and their families to identify social, emotional, and financial needs.
• Develop personalized care plans in collaboration with interdisciplinary team members, including nurses, physicians, and therapists.
• Provide counseling and support services to patients and families to enhance coping skills and improve overall well-being.
• Assist patients and families in accessing community resources, such as financial assistance, support groups, and transportation services.
• Advocate for patients' rights and ensure they receive appropriate care and services.
• Document patient assessments, interventions, and progress in accordance with regulatory standards and agency policies.
• Participate in interdisciplinary team meetings to review patient progress and adjust care plans as needed.
• Collaborate with other healthcare professionals to coordinate patient care and ensure continuity of services.

Job Type: Part-time

Pay: $80.00 - $130.00 per hour
