RN or the LVN team leader MUST be knowledgeable and experienced in the following:

Assigning Care team: RNCM, MSW, SC, CHHA

Scheduling: Assigning Admissions/LVN visits

Coordinating Afterhours/weekend on call schedules

Take part in Administrator on call rotation

Run IDG meetings

New Admit "tuck in" calls

Coordinate hospital discharges with hospital/intake coordinator

Audit/monitor timely documentation to ensure compliance

Audit/monitor that careplans are updated and individualized

Review referrals for hospice appropriateness

Attend careplan meetings when needed

Provide knowledge/support and guidance to field staff

Address customer care issues

Review level of care criteria and orders

Shadow/mentor field visits with the staff

Assist in completing death worksheets
