Supported Living Group LLC is Hiring a Home Health Care / Brain Injury Support Staff Near Southington, CT

Qualifications:

  • A passion to improve lives
  • A high school education (or GED)
  • Valid Driver's License and reliable transportation
  • 18 years or older
  • Personal cell phone
  • 1 to 2 years experience providing support to Brain Injury Survivors in a Community Based setting

We're hiring for multiple positions!

Available positions:

  • Companion positions starting at $15.69/hr
  • Pre Vocational positions starting at $16.00/hr
  • ILST positions starting at $18.00/hr

Services we provide:

Companion (Comp/PCA): Services are provided in accordance with therapeutic goals. These services include supervision, socialization, assistance and/or supervision of meal preparation, housekeeping tasks that are supplementary to the ability of the individual, and other needs.

**Providers must complete a Supplemental application and attend the ACR ABI informational session.

ABI Recovery Assistant I & II (RA): Supports to promote the individual's abilities to maintain and foster community living skills, in accordance with therapeutic goals outlined in the individual's service plan.

Vocational (Pre-Voc/SE): Services provide paid learning and work experience or volunteer opportunities for the client. The individual can develop general job and non-job-task-specific strengths and skills that contribute to employment or employability in an integrated work setting.

Independant Living Skills Training (ILST): SLG's Independent Living Skills Training Services are individually tailored to individual life goals and recovery targets. SLG's credentialed ILST level staff educate, supervise, and assist our ABI Waiver Participants to develop and maintain crucial and beneficial life skills to improve their lives.
