As a Care Navigator RN or SW, you will: •Identify, assess, plan, implement, coordinate, monitor, and evaluate healthcare services to meet an individual patient's specific health care needs in a cost-effective manner. (may include but not limited to home health assessment, telehealth initiation, disease management education, coordinate of post-acute services) •In collaboration with other key Care Team roles, coordinate patient care with patient, family, staff, physicians, and ancillary/community services to promote quality and cost-efficient care for assigned caseload. •Engage physician practices to review gaps in care for patient population and conduct outreach to facilitate closing the gap •Ensure weekly contact with patient (in person or via telephone) to progress towards care plan goals •In collaboration with the Community Health Worker, act as an advocate for the connection with community resources. •Conduct patient home assessment visit when indicated RN Qualifications include: •Graduate of an accredited school of nursing with a minimum of an associate of science degree required, bachelor’s degree preferred •1 – 3 years’ experience in acute care setting at least 1 year of prior experience as case manager, discharge planner or care coordinator desired •Maryland RN licensure •Licensed driver and able to operate a motor vehicle as well as maintain automobile insurance. •Active American Heart Association Basic Life Support (BLS) certification required SW Qualifications include: •Graduate of an accredited school of social work with a minimum of a Master of Social Work degree required •3 – 5 years’ experience in acute care setting at least 1 year of prior experience as case manager, discharge planner or care coordinator desired •Maryland LMSW, LCSW, or LCSW-C licensure •Driver license preferred, must have access to immediate transportation to see patients in the community when needed •Active American Heart Association Basic Life Support (BLS) certification required Work Schedule: Full Time, Monday-Friday #GR8 Nursing At Adventist HealthCare our job is to care for you.
