Job Description
The primary purpose of the Licensed Vocational Nurse is to provide direct care and services to patients in their homes under the supervision of healthcare professionals. The LPN will work with other healthcare professionals in providing quality care and services that support patients’ well-being. The LPN will be responsible for the direct care and services of patients. Responsibilities include but are not limited to:
·Provide skilled nursing care in the resident’s home under the supervision of Registered Nurse
·Administer medications and supplements according to State and Federal regulations within the limits of the licensed facility; assist with dispensing medications, injections and other health services as needed
·Monitor residents’ condition, report changes in parameters, and initiate action as needed
·Participate in training, continuing education, and development programs as assigned by Management
·Coordinate with Management regarding changes in parameters or adverse events in the resident’s home
·Participate in on-call rotation schedule based on availability of service
Required Skills:
·Must have a current Long Beach, CA State License as a Licensed Vocational Nurse (LPN)
·Must have a current CPR certification (BLS) and be able to obtain within the first 90 days of employment
·Must have a positive attitude, good work ethic, and flexibility to work various shifts and overtime as needed
·Must have excellent interpersonal skills and be able to work with all levels of personnel within the facility
·Must be knowledgeable with computer skills including but not limited to Microsoft Office applications (Excel, Word) and computerized billing systems (Carefusion)
·Must have excellent communication skills both written and oral speaking both interpersonally and through phone/email communication for both written communication and face-to-face interaction with residents, staff, family members, physicians, insurance companies, government agencies etc.
