Job Overview:
We are seeking a highly skilled and compassionate Registered Nurse Case Manager to join our healthcare team. As a Registered Nurse Case Manager, you will be responsible for coordinating and providing comprehensive care to patients, ensuring their needs are met and their health is optimized. Duties:
- Conduct patient assessments to determine their healthcare needs and develop individualized care plans
- Monitor and evaluate patient progress, making adjustments to care plans as necessary
- Collaborate with healthcare professionals, including physicians, specialists, and therapists, to coordinate patient care
- Provide education and support to patients and their families regarding treatment options, medications, and self-care techniques
- Administer medications and treatments as prescribed by physicians
- Perform vital sign measurements and other clinical assessments
- Assist with medical procedures as needed
- Document patient information accurately and maintain up-to-date medical records
Skills:
- Strong knowledge of human physiology and the ability to apply it in patient care
- Proficient in conducting patient assessments and developing care plans
- Skilled in monitoring patient progress and making appropriate adjustments to care plans
- Experience in various healthcare settings such as ER, ICU, hospitals, managed care, critical care, or acute care
- Excellent communication skills to effectively collaborate with healthcare professionals and educate patients
- Ability to provide compassionate patient care while maintaining professionalism
Requirements:
- Valid Registered Nurse (RN) license
- Bachelor's degree in Nursing (BSN) preferred
- Minimum of 2 years of nursing experience in a clinical setting
- Case management certification is a plus
