We are seeking a dedicated LPN to join our home health team. As an integral part of our organization, you will provide compassionate and comprehensive care to patients within their homes. Responsibilities:

• Conduct start of care assessments
• Regular nurse visits
• Wound care
• Develop individualized care plans
• Administer medications and treatments as prescribed
• Educate patients and their families on managing illnesses and promoting wellness
• Collaborate with interdisciplinary teams to ensure coordinated and effective patient care
• Monitor and document patient progress and make appropriate recommendations

Qualifications:

• Active LPN license in Florida
• Minimum 1 year of clinical experience, preferably in home health or a related field
• Proficient in clinical assessments and care planning
• Strong communication and interpersonal skills
• Ability to work independently and within a team environment
• Reliable transportation and valid driver's license

Preferred Qualifications:

• Certification in Home Health Care
• Experience with electronic medical records (EMR) systems
