Seeking a dedicated Registered Nurse (RN) for a 13-week contract position in North Charleston, South Carolina. Join our home health facility as a Patient Care Coordinator and make a difference in the lives of our patients. Experience with HomeCare HomeBase and Oasis is required.

Responsibilities:

• Conduct in-home visits to assess patient conditions and provide skilled nursing care.
• Perform diagnostic testing, monitor vital signs, and administer treatments as prescribed.
• Collaborate with interdisciplinary team members to develop and implement patient care plans.
• Communicate effectively with patients and their families, providing education and support.
• Maintain accurate documentation of patient assessments, interventions, and progress.

Requirements:

• Valid RN license in South Carolina.
• Proficiency in HomeCare HomeBase and Oasis software.
• BLS certification from the American Heart Association.
• Minimum 6 months of experience in home health.
• Excellent communication and interpersonal skills.
• Reliable transportation and a valid driver's license.
