Position: Nurse Practitioner (NP) - SNF Patient Care

Location: Fairfield County, CT (Primary SNF Locations in Norwalk and Danbury)

Type: Direct Contract - 52 Weeks Assignment

Schedule: Monday through Friday, 8 am - 5 pm

Are you a compassionate and dedicated Nurse Practitioner seeking an impactful role in Fairfield County, CT? We have an exciting opportunity for you! We are currently seeking a Nurse Practitioner for a 52-week contract to provide exceptional care to patients in short-term SNF stays within Fairfield County, with primary locations in Norwalk and Danbury.

Responsibilities:

• Conduct comprehensive one-time assessments, including physical examinations, medication reconciliation, medical history reviews, ADLs evaluation, nutritional assessments, cognitive evaluations, pain assessments, fall risk evaluations, and social determinants of health assessments.
• Develop and communicate problem lists along with recommended treatment plans for each resident.
• Ensure meticulous and complete documentation, including ICD10 coding.
• Collaborate effectively with the interdisciplinary care team.
• Engage in advanced illness and advanced care planning conversations.

Program Objectives:

Our primary goals include enhancing overall care quality, reducing avoidable hospitalizations, and assisting members in devising care plans aligned with their aspirations. The NP will work closely with the members primary care physician, the member/responsible party, and nursing home staff to deliver primary/preventive care and personalized care coordination.

Qualifications:

• Active NP license in the state of practice, unencumbered by ongoing disciplinary actions.
• Minimum 1 year of NP experience.
• Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification.
• Reliable transportation to conduct assessments in nursing homes, assisted living, and home settings.
• DEA Certification.

Additional Information:

• Mileage reimbursement between locations at the federal rate.
• Standard business hours, Monday through Friday (8 am - 5 pm).
