Description: We are hiring a Registered Nurse for a 13-week contract in Pleasant Hill, CA, to provide compassionate home health care. This full-time position offers an opportunity to make a meaningful impact on patients' lives.

Responsibilities:

1. Conduct thorough patient assessments and develop individualized care plans.
2. Administer skilled nursing care, including wound care, medication management, and patient education.
3. Document patient interactions accurately and timely using electronic medical records.
4. Collaborate with interdisciplinary team members to ensure comprehensive care delivery.
5. Participate in on-call rotation and address after-hours patient needs as required.

Requirements:

1. Active RN license in California.
2. Current BLS certification from the American Heart Association or the American Red Cross.
3. Previous experience in home health nursing required.
4. Preferred proficiency in OASIS and HCHB EMR systems.
5. Ability to work full-time hours, Monday through Friday, with possible on-call rotation.
6. Case management experience preferred.
