- Active RN Licensure
- Minimum of one year recent home health experience
- BLS Certification (American Heart Association)
- Proficient in OASIS documentation and Homecare Homebase (HCHB) system
Responsibilities:
- Collaborate with healthcare professionals to craft and implement personalized home care plans
- Conduct thorough patient assessments to develop tailored care strategies
- Administer medications, provide wound care, and deliver skilled nursing interventions
- Monitor patient progress, liaise with interdisciplinary teams, and maintain communication
- Utilize OASIS documentation and HCHB system to ensure accurate and timely record-keeping
Details:
- 13-week contract position in home health care
- Full-time schedule: 40 hours per week (8:00 AM to 5:00 PM)
- Mileage reimbursement for travel
- Comprehensive benefits package available
