Are you a compassionate and experienced Home Health Registered Nurse (RN) seeking a fulfilling opportunity in Camp Hill, PA? We are currently hiring for a full-time contract position, offering 40 hours per week with flexibility for occasional weekends and on-call duties.

Responsibilities:

• Patient Assessments:

  • Conduct thorough assessments of patients' physical, emotional, and social needs, utilizing Oasis for accurate documentation. Collaborate with interdisciplinary teams to develop personalized care plans.

• Skilled Nursing Procedures:

  • Perform skilled nursing procedures, including medication management, wound care, and IV therapy, adhering to physician orders and best practices in patient care.

• Technology-Driven Documentation:

  • Utilize Homecare Homebase for meticulous documentation of patient care activities, ensuring compliance with regulatory standards and agency protocols.

• Patient and Family Education:

  • Provide tailored education to patients and their families on disease management, medication administration, and proactive health measures, empowering them to take control of their wellness journey.

• Collaborative Care Coordination:

  • Participate actively in interdisciplinary meetings, fostering effective communication and coordination of care to optimize patient outcomes and satisfaction.

Job Details:

• Location: Camp Hill, PA
• Schedule: Full-time, 40 hours per week, Monday to Friday, 8:00 am to 5:00 pm
• Occasional weekends & on-call.

Requirements:

• Valid Registered Nurse (RN) license in the state of Pennsylvania.
• Minimum of 2 years of Registered Nurse experience, with at least 1 year in home health.
• Proficiency in Oasis and Homecare Homebase documentation systems.
• Excellent clinical assessment and critical thinking skills.
• Strong communication and interpersonal abilities.
• Reliable transportation and willingness to travel within designated service area.
• Active American Heart Association or American Red Cross BLS
