• Client: Nationwide Home Health Agency
• Position: Contract Home Health Registered Nurse (RN)
• Duration: 13 weeks with possibility of extension upon successful completion
• Schedule: Monday to Friday, 8:00 am - 5:00 pm, occasional weekends, and on-call (typically once per month)
• Compensation: Negotiable based off experience and territory
• Benefits: Mileage reimbursement, Medical, Dental, Vision, 401k eligibility after 90 days

Job Requirements:

• Active OregonRN License
• Active American Heart Association (AHA) or American Red Cross Basic Life Support (BLS) certification
• Minimum 1 year of experience with Oasis and Homecare Homebase
• Minimum 1yearof home health experience
• Minimum 1 year of hospice experience
• Reliable transportation

Job Duties:

• Provide compassionate end-of-life care to hospice patients and their families in their homes, ensuring dignity and comfort.
• Perform thorough assessments to identify physical, emotional, and spiritual needs, and develop individualized care plans.
• Administer medications and treatments as prescribed, and manage symptoms effectively to promote quality of life.
• Offer emotional support, counseling, and guidance to patients and families, fostering a supportive and caring environment.
• Collaborate with interdisciplinary team members and community resources to coordinate holistic care and support services.
• Document patient assessments, interventions, and outcomes accurately and timely, adhering to hospice regulations and guidelines.
