• Client: Nationwide Home Health Agency
• Position: Contract Home Health Registered Nurse (RN)
• Duration: 13 weeks with possibility of extension upon successful completion
• Schedule: Monday to Friday, 8:00 am - 5:00 pm, occasional weekends, and on-call (typically once per month)
• Compensation: Negotiable based off experience and territory
• Benefits: Mileage reimbursement, Medical, Dental, Vision, 401k eligibility after 90 days

Job Requirements:

• Active PennsylvaniaRN License
• Active American Heart Association (AHA) or American Red Cross Basic Life Support (BLS) certification
• Minimum 1 year of experience with Oasis and Homecare Homebase
• Minimum 1yearof home health experience
• Minimum 1 year of hospice experience
• Reliable transportation

Job Duties:

• Provide compassionate and dignified end-of-life care to hospice patients in their homes.
• Conduct thorough assessments of patients' physical, emotional, and spiritual needs.
• Develop and implement individualized care plans in collaboration with the interdisciplinary hospice team.
• Administer medications and treatments as prescribed to manage pain and symptoms effectively.
• Offer emotional support and counseling to patients and their families during the end-of-life journey.
• Coordinate with community resources and other healthcare providers to ensure comprehensive care for patients and families.
• Document care provided accurately and timely in accordance with hospice regulations and guidelines.
