Job Details:

• Client: Nationwide Home Health Agency
• Position: Contract Home Health Registered Nurse (RN)
• Duration: 13 weeks with possibility of extension upon successful completion
• Schedule: Monday to Friday, 8:00 am - 5:00 pm, occasional weekends, and on-call (typically once per month)
• Compensation:$50-$55 per hour (dependent upon experience)
• Benefits: Mileage reimbursement, Medical, Dental, Vision, 401k eligibility after 90 days

Job Requirements:

• Active CaliforniaRN License
• Active American Heart Association (AHA) or American Red Cross Basic Life Support (BLS) certification
• Minimum 1 year of experience with Oasis and Homecare Homebase
• Minimum 2years of home health experience
• Reliable transportation

Job Duties:

• Conduct thorough patient assessments and develop tailored care plans to meet individual needs.
• Administer skilled nursing care, including wound care, medication management, and symptom control, in the comfort of patients' homes.
• Collaborate with interdisciplinary team members to ensure holistic patient care and continuity of services.
• Utilize OASIS and Homecare Homebase software for accurate documentation, care coordination, and communication.
• Educate patients and their families on disease management strategies, medication regimens, and self-care techniques.
• Monitor patient progress, evaluate outcomes, and adapt care plans as necessary to promote optimal health and independence.
