Position Details:

• Contract Type: 13-week contract, full-time
• Hours: 40 hours per week, Monday-Friday, 8am-5pm with occasional weekends and on-call
• Location: Mount Juliet, TN
• Pay: Negotiable based on experience and territory, weekly pay, stipend pay packages available for travelers
• Benefits: Medical, dental, vision, 401k after 90 days, mileage reimbursement

Position Requirements:

• Active Tennessee (TN) Registered Nurse (RN) License
• Active American Heart Association or American Red Cross BLS
• Minimum 1 year of home health experience
• Minimum 1 year of Oasis and Homecare Homebase experience
• Reliable transportation
• Joint Commission onboarding upon offer

Position Duties:

• Conducting comprehensive patient assessments and collaborating with interdisciplinary teams to develop individualized care plans.
• Administering medications, treatments, and therapies as prescribed by healthcare providers.
• Monitoring patients' progress, evaluating response to treatment, and adjusting care plans as necessary.
• Educating patients and their families on disease management, medication adherence, and lifestyle modifications.
• Utilizing Oasis and Homecare Homebase software for accurate documentation and effective communication with the healthcare team.
