Sun River Health is currently seeking a full-time Licensed Physical Therapy Assistant to join our busy team at our Bronx Boulevard, Bronx, NY location.

Under the supervision of a Physical Therapist, this position promotes and maintains patient health by providing physical therapy services.

ESSENTIAL FUNCTIONS:

• Assist in Assessments: Assist the Physical Therapist in carrying out treatment plan and gathering objective.
• Treatment Support: Implement treatment plans as directed by the Physical Therapist, including exercises, modalities, and mobility training.
• Monitoring performance and report any changes or concerns to the supervising Physical Therapist.
• Documentation: Maintain accurate and timely documentation of resident progress and any interventions provided. Comfortable and knowledgeable with Point of Service (POS) care delivery and documentation. Maintain detailed and up-to-date documentation, treatment plans, daily and progress notes.
• Collaborate with the interdisciplinary team, following the guidance of the Physical Therapist and in cooperation with other medical staff.
• Clean treatment areas and set up therapy equipment, assist patients moving to or from a therapy area.
• Adhere to professional standards, policies and procedures, federal, state, and local requirements.

EDUCATION/EXPERIENCE:

• Active NY State licensure as a Physical Therapy Assistant.
• CPR/BLS certification.

Bilingual Spanish is preferred.
