Job Description
Sun River Health provides the highest quality of comprehensive primary, preventative and behavioral health services to all who seek it, regardless of insurance status and ability to pay, especially for the underserved and vulnerable. Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Duchess, Ulster, Sullivan, Columbia and Westchester County. Sun River Health is currently looking to fill a full time MAT Program Manager position at our busy Coram, NY site.
The MAT Program Manager is a part of multidisciplinary team within Sun River Health who acts as a liaison to community resources, supports the MAT teams and assists with MAT program operations. Coordinates Naloxone trainings for staff and the community. Coordinates with MAT teams and Informatics to assure appropriate data is captured in eCW for grants and quality reporting. Attends MAT QIS, case conferences, and other team meetings as appropriate. Manages operations and Naloxone distribution.
Essential Functions:
  • Works collaboratively with the site Nurse Manager to provide support and guidance for MAT RN care managers, including managing client escalations related to care and maintaining up-to-date clinical competencies of staff.
  • Coordinates scheduling and cross-coverage for RN-Care managers.
  • Supports hiring, orientation, and coordination of training for new clinical staff on MAT services and overdose prevention programs.
  • Oversees NYC grant operations and completes all reporting requirements.
  • Promotes program operational consistency in following protocols and policies throughout NYC Region.
  • Act as a MAT liaison with community resources as needed participating in community meetings, providing outreach, treatment coordination, and program collaboration.
  • Collaborates with interdisciplinary team members to develop, implement, and evaluate patient care through team huddles and case conference.
  • Participates in the MAT Quality Improvement program meetings including identifying quality issues and opportunities for improving quality of care.
  • Maintains leadership of the SRH NYC Opioid Overdose Prevention Program including: facilitates organizational trainings, completes program reporting, maintains a registry of staff members trained to distribute Narcan, tracks and reports naloxone use, coordinates with OOPP Program and Medical Director to address program issues, and oversees organizational stock and ordering of supplies.
  • Attends substance use and related training.
  • Participates in MAT program initiatives or other duties as assigned.
Education/Experience:
Bachelors Degree is required, Masters Degree is preferred
Minimum 2-4 years related experience is required
Active CPR Certification is required
CASAC is preferred
