Description

Position Summary:

Interprets physician prescriptions and medication orders. Acts as a drug information resource to patients, medical staff and ancillary department staff. Compounds and dispenses prescribes medications and other pharmaceuticals for patient care by performing the related duties. Requires on-call schedule.

Position Responsibilities:

Clinical / Technical Skills (40% of performance review)

  • Reviews prescriptions issues by physicians or other authorized prescribers to assure accuracy and determines formulas and ingredients needed.
  • Monitors all orders entered in to the CPOE (Computer Physician Order Entry) patient profiles in order to be sure that accuracy, drug allergies, drug interactions, duplicate therapy, rational drug therapy and incompatibles may be realized, and better patient care received.
  • Ensures that the Medication Dispensing System is stocked, and formulary medications are available.
  • Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
  • Directs pharmacy services staff engaged in packaging and labeling pharmaceuticals.
  • Answers questions and provides information to physicians, nurses, patients and other healthcare professionals on the appropriate use of medications, drug interactions, side effects, dosage, storage, etc.
  • Assists in teaching programs (departmental, medical, nursing, community)
  • Follows policy and procedures of the department and of the hospital.
  • Maintains established procedures concerning quality control, security of controlled substances and disposal of hazardous waste.
  • Enters data into the computer system (Pharmacy programs-such as patients, prescribes medications and cost, and maintains patient profiles and charge systems.
  • Reviews and monitors drug therapy that requires dosing adjustments as authorized by the physicians.
  • Provides pharmaceutical care to all patients from children age 5 through adulthood.
  • Identifies and addresses psychosocial, cultural, ethnic and religious/spiritual needs of patients and their families.
  • Assists the pharmacy director on special assignments.
  • Supervises at least one to two (1-2) members of Pharmacy staff daily.
  • Assists the pharmacy director with responsibilities in accordance with the organizations’ missions, policy, and any applicable laws.
  • Demonstrates ability to apply mathematical operations to such tasks as dosage calculations, formulation of pharmaceutical preparations and data analysis.
  • Ensures that pharmaceutical preparations are sterile, properly packages, labeled and stored, and not outdated.
  • Dispenses drugs, chemicals, and pharmaceutical preparations including all narcotic drugs and maintains their perpetual inventory
  • Inspects all pharmaceutical supplies on all services.
  • Maintains an approved stock of antidotes and other emergency drugs.
  • Specifies both quality and source for the purchase of all drugs in conjunction with central bids.
  • Performs all scheduled controlled substance inventories as prescribed by SUN Behavioral, federal, and state law, including a monthly controlled substance inventory and back-counting for all CII transactions.
  • Maintains adequate control over the requisitioning and dispensing of all drugs and pharmaceutical supplies.
  • Assists in implementing the decisions of the Pharmacy and Therapeutics Committee.
  • Assist in the preparation of periodic reports on the progress of the department for submission to the administration of the hospital.
  • Maintains State and Federal required records so as to maintain adequate accountability and control of all medications including controlled substances.
  • Provides direction in accordance with issues identified regarding patient satisfaction.
  • Provides in-depth pharmacologic and pharmacokinetic consults (verbal or written) when requested or when appropriate
  • Coordinates pharmacy activities with other ancillary professional departments
  • Assumes responsibilities for the department's continuous, effective operation and for the improvement of its performance.
  • Perform other duties as required

Qualifications


Education

  • Required: Graduate of a College or School of Pharmacy accredited by the Accreditation Council for Pharmacy Education. Current Pharmacist licensure from the state of employment. CPR certification, which may be obtained in orientation.
  • Maintains education and development appropriate for position.

Experience

  • Preferred: A minimum of three (3) years of hospital experience or one (1) year experience in behavioral healthcare setting.
