We are seeking a dedicated and compassionate Peer Support Specialist to join our team. As a Peer Support Specialist, you will provide assistance and guidance to individuals in need of extra support during their recovery journey.
Duties:
Provide intake and screening, as needed, to new clients. Provide outreach to the community; assist in setting up partnerships with community resources to connect clients with including housing, employment, and recovery support meetings. Develop recovery plans to support clients as they transition to the community, based on the wants, needs, and interest of the participants. Identify mechanisms to increase engagement and support to family members. Maintains accurate records as required by DAODAS, Medicaid and agency policy/procedures, as assigned. Maintains certification necessary for the position and provides evidence of all required training to the personnel department in a timely manner. Use recovery-oriented practices such motivational interviewing. Demonstrates proficiency in use of computerized data systems utilized by the agency. Able to provide outreach and screen clients for entry into MAT services. Good working knowledge of MAT medications and their benefits (over time). Compliance with all Sumter Behavioral Health Services Policies and Procedures. Provides after hours and emergency coverage as needed.
