Summit Healthcare Association is Hiring a Behavioral Health Consultant - SHMA Behavioral Wellness Near Show Low, AZ

The following information is designed to outline the functions and position requirements of this job. It does not identify all tasks that may be expected, nor address the performance standards that must be maintained.

The behavioral health consultant (BHC) is a behavioral health professional who provides ongoing therapy, case management, and referral services to individuals, groups and/or their families in an outpatient setting for patients of Summit Healthcare Medical Associates (SHMA). The BHC is responsible for providing diagnostic assessments, therapy, and support services for a diverse caseload including adults, adolescents, children and families experiencing mental health issues. The BHC assists clients with symptom reduction and uses evidence-based models for treatment.

Essential Functions / Major Responsibilities:

  • Provides evidenced based and culturally competent behavioral health treatment and interventions.
  • Develops a comprehensive, individualized treatment plan and goals with the client; updating and modifying as needed.
  • Establishes positive, trusting rapport with clients.
  • Involves and advises family members when appropriate.
  • Performs telehealth interventions and group therapy services.
  • Use of DSM-5 for diagnosis and evidenced based behavioral health assessment tools.
  • Conducts ongoing assessments of client progress.
  • Adds to the multidisciplinary care plan with patient and/or family input.
  • Provides psycho-education for patients during individual or group visits.
  • Triages patients with severe or high-risk behavioral health problems and refers such patients to community resources and/or additional behavioral health services.
  • Conducts risk assessments as indicated.
  • As needed, provides resources to meet financial, transportation, housing and other needs.
  • Maintains thorough records of client meetings and progress, submitting documentation and superbills into the electronic medical record in a timely manner to comply with all administrative regulations.
  • Maintains strict confidentiality guidelines regarding all patients, conversations and referrals in line with local, state and federal statutes, and organizational policies.
  • Respects and upholds patient rights, demonstrates professional integrity, discloses all potential or perceived conflicts of interest when appropriate and accepts responsibility and accountability for professional competence.
  • Secondary Functions:
  • Monitors outcomes and develops process change to improve care.
  • Participates in staff meetings, hospital wide forums, and seminars as recommended by supervisor.
  • Reviews department and hospitalwide policies and procedures annually.
  • All other duties as assigned.

  • Specific Job Skills & Mental Activities:
  • Able to establish and maintain effective working relationships with clients and their families.
  • Operational knowledge of equipment including: fax, printers, copy machine, phone systems, and commonly used hospital- wide computer programs (MS office, e-mail, and internet).
  • Must read, write, speak and understand English. Must possess excellent written and oral communication skills.
  • Strong ability to manage time and prioritize tasks. Detail oriented with excellent organizational skills.
  • Expertise in counseling, crisis intervention and behavioral health treatment.

  • Education and/or Experience:
  • Master's degree in counseling, Social Work, or related behavioral health field from an accredited school (required).
  • Current independent licensure (LPC, LMFT, LCSW, Psychology) in the state of Arizona (required and must be maintained throughout employment).
  • Minimum of 2 years' experience working in a clinical setting (required).
  • AZ finger print clearance card (Application completed within 14 days of hire date; required)
  • BLS/CPR certification (preferred upon hire; required within 30 days of hire and must be maintained throughout employment.)
  • Basic computer skills (required).

  • Physical Demands & Job Conditions:

Exert up to 20 lbs. of force occasionally, and/or up to 10 lbs. of force frequently, and/or a negligible amount of force constantly to move objects. Physical demands are in excess of those of sedentary work. Light work usually requires walking or standing to a significant degree. The worker is exposed to extensive close work, extensive computer work, constant standing, and encounters with upset individuals.

Physical motions include finger dexterity, standing, walking, stooping, talking, reaching, feeling, sitting, bending, kneeling, grasping, listening/hearing, handling, lifting up to 10 pounds, and repetitive motions of the hands, wrists, and feet.

This is a safety sensitive position.

Involves no regular exposure to blood, body fluids, or tissues, and tasks that involve exposure to blood, body fluids, or tissues and are not a condition of employment.
