Full-time positions (32 hours per week) include medical vision, and vacation benefits.

Part-time positions are no fewer than 24 hours per week.

CURRENTLY SEEKING:

QUALIFICATIONS:

• Valid California LVN license
• Valid California driver license
• Ability to communicate effectively both verbally and in writing.
• Follows nondiscrimination laws and conduct in the performance of duties
• Good organizational skills; ability to prioritize workload and work independently.
• Must comply with drug-free workplace guidelines.
• Current First Aid certification/CPR certification.
• Current Tuberculosis Test Results (annual requirement).
• First Responders and Veterans experience a must

PATIENT CARE DUTIES:

• Direct patient care including the reporting of observed changes in mental status and/or physical condition, response to emergencies, safety risks
• Availability and timeliness in terms of 24-hour coverage for consultation and emergencies and, when unavailable, informs appropriate individuals
• Coordination with attending physicians to ensure all admission orders are addressed and appropriate ancillary assessments and services are provided
• Completes documentation in an efficient and timely manner, including nursing assessments, transcription of M.D.s' phone orders, appropriate policies and procedures, medication administration sheets, progress relative to the client's treatment plan
• Inclusion of physical health services and other related issues in clients' treatment plans
• Monitor client self-administration of medications
• Responsible for medication counts, tracking and destruction and proper documenting
• Ensure secure control of medications at all times
• Prepares and documents any offsite medications (during client outings)
• Conduct observations and vitals checks, monitors clients regularly and is aware of client whereabouts at all times
• Complete shift report and incident reporting when needed
• Conduct client testing (urine screening, breathalyzer, etc.)

ADMINISTRATIVE DUTIES:

• Properly QA client medication sheets, to determine clients are receiving appropriate medications and in a timely manner, including proper destruction of medication.
• Development and implementation of quality assurance for infection control, nursing, pharmaceutical, ancillary, and laboratory services
• Attendance and participation in terms of treatment planning conferences and professional staff meetings
• Orientation and training of staff regarding pharmaceutical and physical health services and infection control
• Performance in relation to ethics and professionalism
• LVN's overall performance and competence in terms of consistency with education, training, experience and reflective of current standards of practice/care
