Summit Community Care Clinic is Hiring a Supervisor of Health and Wellness Near Frisco, CO

The Supervisor of Health and Wellness will promote, maintain, and improve individual and community health by supporting the Health and Wellness team. They will support their patients in adopting healthy behaviors and connecting with resources to address social determinants of health.

The Supervisor of Health and Wellness may assist in developing and implementing health education programs, brief interventions, and research and develop health education materials. They will facilitate related support groups, implement health education courses, and conduct outreach programs. The supervisor of Health and Wellness may facilitate projects or activities related to attaining or maintaining accreditation, conduct or participate in research projects related to health and wellness issues, and provide skill training related to health and wellness issues.

Supervisor Essential Functions:

  • Program coordinator for Wise Women Program, Social Determinants of Health, and Tobacco Cessation Work and other program duties as assigned.
  • Responsible for guiding the growth and oversight of the health and wellness programs, supervises the health and wellness coaches and the behavioral health navigators.
  • Supervises the health coach and behavioral health navigators.
  • Monitors program and adapts care delivery to ensure patient access to high-quality, evidence-based care.
  • Develops and monitor essential activities of the health and wellness team, document significant events, and provide analysis of reported data.
  • Contributors to community outreach and collaboration with community partners to ensure resource coordination is properly provided
  • Oversees the Unite Us platform, One Degree platform for Social Determinants of Health
  • Responsible for Team schedule, timesheet approvals, PTO requests, coverage issues, and HR conversations.
  • Point of contact for updates, knowledge, and direction for the health and wellness team
  • Oversight of training needed for the health and wellness team.
  • Supervises their direct reports with regular check-in meetings with goal settings for individual and team growth.
  • 15-20 hours of administrative time to complete.

Health and Wellness Coach:

  • Management of patient caseload for nutrition, healthy living, tobacco cessation, and social resources.
  • Documents all encounters and case management information in electronic health records.
  • Uses resources to anticipate, address, and overcome barriers to care and to guide patients through the health care system.
  • Utilizes evidence-based practices and materials when creating resources for patients.
  • Organizes, coordinates, facilitates, and participates in the county and statewide outreach events to increase awareness of our programs and services.

Additional Requirements:

  • Grants are a vital part of Care Clinic revenues and directly or indirectly support all positions. This role may have responsibilities that are closely associated with grant deliverables. Grants and grant responsibilities may change over time.
  • Complete all required trainings as designated by HR and the position supervisor.
  • Adhere to all policies within the organization and those outlined within the SCCC Employee Handbook.
  • Comply with all SCCC Vaccine Mandates.
  • Willingness to travel and work at other SCCC locations as determined by the supervisor.
  • Comply with all applicable Credentialing and Privileging requirements timely.

  • Must participate in quality assurance/ improvement activities when applicable.

Education/Experience:

Must Haves to be successful:

  • Bachelor’s Degree or equivalent.
  • Health and Wellness Coach Certification required; board certification preferred.
  • At least 1 year of experience in a clinic setting or a combination of education and experience.
  • CPR certification.
  • Ability to read, write, and clearly speak English and at least a basic understanding of the Spanish Language.
  • Excellent communication and critical thinking skills with high attention to detail and quality.
  • Embody high emotional intelligence with a patient-centered, eager-to-help approach.
  • Ability to work autonomously.
  • Ability to document concisely, accurately, and in a timely manner.
  • Ability to understand state/federal/agency regulations.
  • Able to handle a variety of duties that may be interrupted or changed by immediate circumstances.
  • Ability to work Monday through Friday (MST).

Nice to Haves:

  • Ability to speak Spanish fluently is preferred.

The work environment in a healthcare setting can be fast-paced and involve working directly with patients and healthcare professionals. The individual may also work in a clerical setting.
