The Case Manager is responsible for the overall coordination of a client’s clinical and programmatic needs. The Case Manager will ensure that communication between the family, funding source representatives, clinical team, and client is open and ongoing to facilitate the flow of necessary information.

JOB RESPONSIBILTIES

• Serve as liaison between the assigned clients, clinical team, family, funding sources, supervisory staff, direct care staff and the community.
• Responsible for on-going contact with families on a weekly, bi-weekly and as needed basis to keep family informed of client progress and any issues that may arise.
• Responsible to identify and manage client banking needs, including paying bills, review of bank statements, spending money. Document transactions according to SRI financial requirements to assure all monies are properly accounted for.
• Responsible for scheduling client medical appointments/follow-up appointments and informing clinical team and staff.
• Monitor DME needs of clients: obtain appropriate approvals; order new equipment; request repairs.
• Meet with clients on an individual basis as needed to address need, problem or programmatic concern.

JOB QUALIFICATIONS

• Bachelor's degree in field related to health care or human services.
• Two years' experience working with TBI or special needs population.
• Mus have at least 1 year of case manager experience.
• Valid Driver's License and own reliable vehicle.

• Valid Driver’s License
• CBIS or attendance in the next class
