Suburban Home Health is Hiring a Home Health Aide Near Noblesville, IN

Suburban Home Health is a jointly owned venture between Hancock Health, Riverview Health, Witham Health Services, Margaret Mary Health and Major Health Partners. Job Description

The Home Health Aide/CNA provides direct patient care to adult and geriatric patients in the home setting. 

  • Provides personal care activities of daily living such as bathing, shampooing, oral hygiene, skin care/nail care, shaving, and dressing.
  • Communicates changes in patient status to the supervising Registered Nurse (RN) in a timely manner.
  • Reports patient complaints to the RN
  • Creates and submits legible, accurate and timely documentation.
  • Performs duties in compliance with agency policies and procedures and local, state, and federal regulations.
  • Demonstrates attention to details for patient's safety and safety of their home environment.
  • Reports unsafe conditions in the home.
  • Practices effective infection control and adheres to infection control policies and procedures.
  • Practices safe lifting and transferring of patients.
  • Communicates clearly and positively with patients, families, team members and others. 
  • Displays a caring, patient’s first behavior.
  • Keeps patients and/or family informed regarding visit time and schedule changes.
  • Attends in-service programs to meet compliance requirements
  • Maintains patient confidentiality in compliance with HIPAA

Basic Qualifications

  • Must be a Certified Nursing Assistant (CNA) and/or possess current Home Health Aide registrar in the state of Indiana. 
  • Must have high school diploma or equivalent
  • Must possess valid driver’s license and automobile insurance in the state of Indiana
  • Must be able to provide own transportation to assignments.
  • Ability to work independently
  • Ability to work in and support a team environment.
