Description

GENERAL DESCRIPTION OF POSITION:

Physicians role is vital in the health care delivery system. This position requires adherence to PCMH principles, policies, and procedures. Delivers safe and high quality medical care. Assesses medical history and physiological, psychological, and behavioral factors to determine the cause (s) of a disease, disorder, or injury; developing plans for treatment; and administering specialized medical treatments and procedures as well as preventive. Participates in organizational committees and performance improvement activities. Reports to the Chief Medical Officer. When assessing the patient, the age of the patient is taken into consideration. Supports the vision and the mission of the Clinic at all times and ensures the patients have a pleasant and satisfactory experience in the manner health care is delivered, managed, coordinated with both internal and external entities and in which the patient participates in the decision making. As appropriate, provider will be providing services to patients by any of the following methods: telemedicine, telehealth, telephonic.

ESSENTIAL JOB FUNCTIONS: (with or without accommodations)

Professional competence: Provides evidence based safe patient care as demonstrated by peer review and chart reviews. Conducts physical examinations and diagnostic evaluations of patients and obtains medical histories. Orders, performs, and interprets diagnostic tests and prescribes, recommends, and administers treatment using the clinics approved electronic system Monitors ongoing medical treatment, reviews diagnostic studies, conducts clinical assessments of abnormal results, and follows-up with patient or refers them as appropriate. Informs Chief Medical Officer of unusual or difficult medical problems. If on a part-time basis, will continue to receive urgent calls from clinic employees after leaving the clinic unless on vacation or sick leave. Maintains a minimum panel of 1,000 patients.

Management of patient and support staff: Physician builds an ongoing relationship with patients and provides continuous and comprehensive care. Physician provides the patients healthcare needs or arranges care with other qualified professionals to meet the patients acute, chronic, and preventive care needs. Provides patient focused care and the patient is supported in disease management and participates in the decision making of their care and patient provides feedback to the care team care is coordinated and integrated across all elements of the healthcare system and the patients community. Fosters collaboration between care internal and community providers. Physician directs and leads a team of individuals that collectively take responsibility for ongoing patient care and meeting the varied needs of their patient population. Welcomes walk in patients and works with support staff to address needs of patients not in their panel.

Quality Performance Improvement: Physician uses establish clinic systems to maximize the information technology through performance reporting, clinical-decision support for clinicians, patient education, and online communication. The physician accepts accountability for numerous ongoing quality improvement activities and participates in a certification program. Physician uses patient population data to work with the patient, clinical team, and others to improve healthcare outcomes such as HRSA and meaningfulness healthcare indicators. Physician enhances access to care through open scheduling, expanded hours, and other innovative options for communication between patients and clinic staff. Access may be in person, by telephone, or through a secure, web-based patient portal. Monitors respective performance to ensure all clinical indicators are met and quality services are being provided. Welcomes changes as healthcare transforms.

Patient Care Documentation: Uses disease registries, information technology, and health information exchanges so that patients get the care they need in a culturally and linguistically appropriate manner. Electronic Medical Records are complete and ready for billing as per Clinic policy. Codes identified are consistent with the services provided. All entries are completed and signed.

Professional Development Maintains admitting privileges as required. Accepts E.R. call, unassigned patients and consultation. Conducts home visit within the scope of the privileges granted by the Clinic. Participates in Performance Improvement activities, including peer review, medical record quality, laboratory, x-ray usage, pharmaceutical formulary, and drug usage. Reviews and approves protocols and standing orders for physicians, mid-level practitioners and nurses. Assists with the supervision and teaching of medical students, residents and other professional students. Participates in research activities. Maintains professional courtesy. Required documentation (credentialing, license, certifications, CME) are timely. Responsible for their own safety as well as the safety of others.

Qualifications


KNOWLEDGE, SKILLS, AND ABILITIES: (use of equipment, job related knowledge, language, etc.)

Skilled in excellent customer and communication skills

Ability to read, write and speak in a clear, accurate and professional manner; includes active listening skills and understanding of patients needs

Proficient in beginning/intermediate computer skills and typing.

Excellent follow through skills, multi-tasking, prioritization and attention to detail.

Knowledge of the Chronic Care Model and Patient-Centered Medical Home Model (PCMH).

Must be able to apply critical thinking skills and make sound judgment at all levels throughout the patients continuum of care and make necessary referrals on behalf of Patient Provider or Caregiver.

Knowledge of regulatory requirement, health plan contracts, governmental benefits and community resources.

Ability to apply critical thinking skills while performing daily responsibilities

Excellent interpersonal skills reflecting clarity, diplomacy and communicates accurately and effectively with all levels of staff and management.

Ability to speak, understand and write Spanish and English.

Knowledge of SC policies and procedures.

Knowledge of advanced principles of health promotion, prevention, and motivation.

Knowledge of documentation of individualized care and treatment plans.

Knowledge of protocols in general and an ability to quickly gain specific knowledge of clinics operational protocols.

Knowledge of the Texas Medical Practice Act

Skill in the care and treatment of patients; and in the use of medical diagnostic and treatment tools and equipment.

Ability to maintain an effective and professional working relationship with the public and co-workers.

Able to bend, stretch, stoop, stand, sit, and lift.

Able to lift 20 lbs.

Ability to examine, diagnoses, and treat physical or mental disorders; to interpret laboratory analyses and x-rays.

Ability to maintain a positive work environment.

Ability to work in a fast paced environment.

Ability to maintain a flexible work schedule.

Ability to maintain client and office confidentiality.

Ability to fully comply with the enhanced infection control requirements of the clinic.

EXPERIENCE DESIRED/CERTIFICATION REQUIRED: Must be licensed as a Medical Doctor by the State of Texas. Must be certified or eligible for certification by the American Board of Medical Specialties. Experience in working with the public. Must be current with BLS/CPR. This position requires an initial N95 mask fit testing and annual test fitting thereafter unless there is a significant change during the year that requires retesting prior to the annual fit testing.

SPECIAL INSTRUCTIONS: Due to the nature of SC Services, it may be necessary for employees to work extended hours or other variations of the usual shift to ensure adequate care to patients and to maintain service to the community. CPR /BLS certified
