GENERAL DESCRIPTION OF POSITION:

Therapist will perform all duties to ensure the patients have a pleasant and satisfactory experience in the manner in which their health care is delivered, managed, coordinated with internal and external entities. Utilizing a “person centered approach” that encourages self-determination with dignity and worth of the person, the clinician will provide individual, family and group psychotherapy. As a member of the clinical team, this position requires total support to the PCMH principals, policies, and procedures. The clinician will demonstrate clinical knowledge and skills to assess, diagnose, and treat mental, emotional, and behavioral disorders in adults, adolescence, and children. The clinician will apply therapeutic modalities and methods to the treatment and prevention of psychosocial dysfunction, disability or impairment, including emotional and mental disorders. The clinician will conduct a brief intervention assessment, biopsychosocial assessment, treatment planning and discharge planning with measureable and feasible goals for the patient. The clinician will utilize screenings for ADHD; Depression, Anxiety, Conduct Disorders, amongst others. The treatment interventions utilized will assist SC patients in behavior change to maintain a healthy lifestyle and quality of life.

Focus will be in working with children, adolescents, and adults; ages of 2 through adulthood. The clinician expectation is to see 6-8 patients a day with completing of documentation timely.

As appropriate, provider will be providing services to patients by any of the following methods: telemedicine, telehealth, in office visits; and telephonic. The Clinician will adhere to COVID protocols and state and county guidance. The clinician will have direct contact with the SC patients requiring the use of N95 and follow CDC guidelines for infection prevention and control.

ESSENTIAL JOB FUNCTIONS: (With or without accommodations)

Patient Relationship: Provides individual, group, and family counseling to SC patients experiencing mild to chronic mental health disorders to include substance use disorders. Meets with parents as necessary to enhance the child’s therapy. Uses communication and empathy to develop rapport with the patient and their families. Provides emotional support to patients and arrange health care or other services through the referral process.

Management of Patients: Serves as a liaison and establishes linkages between community mental health providers, SC primary care physicians, patients, and care managers in developing a network of mental health referral sources. Motivates patients and families, giving support, showing interest, conveying understanding in an effort to have patients seek and address mental health care. Report first-hand accounts of situations reported by patients directly to CPS (Child Protective Services) or APS (Adult Protective Services) according to state regulations. May be asked to prepare monthly reports or participate in pilot programs as needed and serve as a field instructor for students by supervising interns as appropriate to the level of training.

Manages Patient Information: Adheres to all HIPAA regulations in and outside of the Clinic. Communicates patient information as requested to enhance patient services for continuity of care. Maintains patient registries in which data will be logged, all treating providers identified, consulting psychiatrist identified as well as treatment attendance dates. In addition, changes in treatment, attempted contacts, actual patient treatment compliance, and response to treatment, treatment outcomes, and discharge will be documented. Will provide services to Medicaid and Medicare patients adhering to those requirements. Works with administration to meet the strategic goals of the organization as it relates to HRSA, grants, etc.

Team Communication: Functions as a vital member of the patient care interdisciplinary clinical team. Serves as the clinic’s expert in mental health services. Assists in the identification and coordination of mental health services for SC patients. Approaches patient care as a “team” involving clinical and non-clinical staff (e.g. physicians, mid-levels, educators, appointments staff) to achieve patient care objectives. Participates in team conferences and in peer review functions. May develop or coordinate educational sessions for peer development and/or increase knowledge of community resources. Submits required documentation in a timely manner (leave requests, credentialing requirements, license renewals, certifications, CEU attendance, etc.).

Demonstration of Safe and Professional Conduct: For all assigned patients, will work with all designated physicians and update them on a periodic basis. Will conduct face-to-face and telephone interactions with patients to ensure maximum treatment effectiveness. Refrains from informal communications with patients outside of SC. Submits required documentation in a timely manner (credentialing requirements, license renewals, certification, CNE attendance, etc.). Maintains a friendly and safe environment for self and others. Responds to emergency situations as per SC training. Refrains from texting or speaking on cell phone while on duty. Participates in performance improvement teams and follows all the clinic policies and procedures. Attends work on a regular and predictable schedule in accordance with clinic leave policy. Performs other duties assigned.

Knowledge of modern social workers practices and methods. Knowledge of electronic medical records.

Knowledge of Medicare regulations and the treatment of substance use disorders

Knowledge of SC policies and procedures.

Knowledge in providing effective Customer Service on a daily basis.

Knowledge of computer software (Microsoft Word, Excel, etc.) programs.

Ability to form and maintain effective relationships with clients.

Skilled in the use of a computer.

Ability to read, write and spell accurately.

Ability to maintain an effective and professional working relationship with the public and co-workers.

Able to bend, stretch, stoop, push, pull and lift.

Able to lift 20 lbs.

Able to write reports.

Able to operate a keyboard, telephone, and other office equipment.

Able to record, prepare and communicate appropriate reports.

Ability to maintain a positive work environment.

Ability to work in a fast paced environment.

Ability to maintain a flexible work schedule.

Ability to maintain client and office confidentiality.

Ability to communicate in both English and Spanish.

MINIMUM QUALIFICATIONS/CERTIFICATES/LICENSES/REGISTRATIONS REQUIRED: Must have Master’s Degree in Social Work from an accredited university. Must be licensed by the Texas State Board of Social Worker Examiners. Licensed Clinical Social Worker (LCSW). Minimum of 1 year working in mental health setting conducting in group and or individual therapeutic sessions. Must be current in BLS/CPR. Bilingual (English/Spanish) preferred.

SPECIAL INSTRUCTIONS: May require contact with individuals with communicable diseases.

Due to the nature of SC Services, it may be necessary for employees to work extended hours or other variations of the usual shift to ensure adequate care to patients and to maintain service to the community. This position requires travel. Employee must provide transportation. If employee operates a personal motor vehicle in the performance of their official duties, the employee must possess a current valid Texas driver’s license for the appropriate type of vehicle and Texas Liability insurance. This position requires that an employee’s driving record be verified with law enforcement to ensure compliance with SC’s driving policy.
