Strong Force Staffing, a leading Recruitment Process Outsourcing (RPO) provider, is currently recruiting for the position of RN Case Manager for a valued client agency in St. George. This vital field role entails direct patient care and case management within the community, aiming to optimize healthcare outcomes. This position offers competitive salary, on-call stipends, and mileage reimbursement. The final hiring decision will be made by our client agency.
Key Details:
· Salary: $82,000 - $99,000 annually
· Benefits: On-call stipends, Mileage reimbursement
· Type: Field role

Responsibilities:
· Assess patient conditions and develop appropriate care plans.
· Maintain comprehensive medical records and report patient status to necessary parties.
· Act as the primary point of contact among patients, their families, and healthcare providers.
· Educate patients and families on health management and preventive measures.
· Coordinate with healthcare professionals to ensure continuous care and efficient case management.
Requirements:
· License: Active RN license in Utah with no restrictions.
· Experience: At least 1 year in a similar in-home healthcare role.
· Skills: Proficient in case management software, excellent organizational and communication abilities.
· Traits: Empathetic, detail-oriented, and capable of autonomous operation in field settings.
