Job Summary:

LPN required:

We are seeking a dedicated and experienced Health and Wellness Coordinator to join our team. As the Health and Wellness Coordinator, you will be responsible for managing and overseeing the wellness programs and services provided to our residents. Your role will involve ensuring the well-being and quality of life for all residents, while adhering to resident rights and promoting a positive and engaging environment.

Duties:
- Develop, implement, and manage wellness programs that meet the physical, emotional, and social needs of our residents
- Supervise and train staff members involved in wellness programs
- Collaborate with other departments to ensure seamless coordination of care and services
- Provide guidance and support to residents and their families regarding wellness options and resources
- Monitor resident progress and make necessary adjustments to wellness plans as needed
- Stay up-to-date with industry trends and best practices in wellness care
- Ensure compliance with all relevant regulations, including HIPAA guidelines
- Maintain accurate documentation of resident assessments, care plans, and progress notes

Qualifications:
- Bachelor's degree in a related field (e.g., healthcare administration, nursing, social work) preferred
- Previous experience in a supervisory role within a healthcare or senior living setting
- Knowledge of hospice care, acute care, medical terminology, anatomy, and home health services
- Strong understanding of resident rights and the ability to advocate for their well-being
- Excellent communication skills, both written and verbal
- Proficient in Microsoft Office Suite and electronic medical record systems
- Ability to effectively manage multiple tasks and prioritize responsibilities
- Strong leadership skills with the ability to motivate and inspire a team
