Description

STRIDE Community Health Center is a 501(c)(3) Federally Qualified Health Center (FQHC) with a 30-year history of providing primary care and other health-related services in a community-based setting. Established in 1989 in response to the primary health care needs of the medically indigent population in suburban Denver, STRIDE Community Health Center has the largest population base of any FQHC in Colorado and provides primary care, dental, pharmacy, behavioral health, health education, care coordination, patient navigation, pharmacy, and outreach and enrollment services in accordance with its mission.

STRIDE’s current service area encompasses Adams, Arapahoe, Douglas, Jefferson, and Park counties and the Cities of Aurora, Arvada, Englewood, Lakewood, Parker, Conifer, and Wheat Ridge.

What will you be doing in this role? A successful candidate will:

Position Summary: To ensure efficient, quality health care services, the Physician provides direct health care for patients in the Refugee Screening environment.

  • Performs medical screening for newly arriving refugees as per CDC guidelines for refugees.
  • Communicate pertinent information to medical providers who perform refugee medical screenings to assure that standard of care and CDC screening guidelines are standard practiced.
  • In collaboration with clinic team members, participates in activities between all care team members at this site, who include, but are not limited to, medical providers, behavioral health providers, business and clinical operations staff, RN, and Refugee Services department. These refugee-care specific team meetings help to develop, implement, update, and document refugee medical screening clinical workflows and program development.
  • Assures that Refugee Screening Clinical Collaborative group meeting information, pertinent to clinical standards, is communicated back to medical providers who involved in refugee care and works with Clinical Site Manager (CSM) to assure that updates are being adhered to and accounted for.
  • Under the guidance of CSM, performs quarterly chart reviews to assure that standard screening guidelines are followed by screening providers, that programmatic protocols remain standard, and that identified diagnoses found at screening appointments are appropriately addressed and followed up.
  • With guidance CSM, works to disseminate CDC outbreak announcements to appropriate teams and determine how medical providers and care team specifically address any outbreak and works with operation team it implements any necessary system changes.
  • participates in activities that may lead to identifying gaps in screening standards and supports day to day trouble shooting related to the medical screening.
  • Assists in the support of Class B chart review (Class B is identified as health-related conditions that may require extensive medical treatment or follow-up) and serves as a backup for these chart reviews.
  • Attends and participates in regularly scheduled screening meetings at the discretion of the CSM.
  • In partnership with Health Education and Nursing Departments, provides expertise and supports elements of the development, implementation, and communication with partners of the Latent Tuberculosis Infection treatment program.
  • In collaboration with RN Refugee Health Community Care Manager and Refugee Services develops and maintains system for scheduling preventive care visits, mental health services and follow up medical appointments or referral to outside clinics for primary and specialty care.
  • This job description is intended to describe the role of a clinical specialist and their role in our Agencies delivery of that specialized healthcare; it is not a supervisory position though. Medical providers will report to their assigned CSM; other team members will report to the supervisor as directed through their teams and/or HR.

Location: Peoria Clinic (crossroads Peoria St. & Baranmor Pkwy. Aurora, CO)

Schedule Offered: Full Time Monday-Friday 8a-5p or 4x10 hours with Monday or Wednesdays off.

Qualifications


Essential Duties/Responsibilities & Job Qualifications:

Education or Formal Training:

  • Doctoral degree in Medicine or Osteopathy from an accredited educational institution or school.
  • Medical residency training from accredited educational institution or school.
  • Board certification in primary care specialty and on-going completion of required trainings

Experience: At least 1 years of specialty clinical experience

Material and equipment directly used:

  • Computer, Microsoft Office products, Electronic Medical Records system and internet.
  • Copier, printer, scanner, fax machine and calculator.
  • Multi-line telephone.
  • Video and telephone conferencing equipment.
  • Other office equipment as needed.

  • Cell phone and Computer, iPad, printer, multi-line telephone systems, fax machine, copy machine, calculator, and all Microsoft Office products including Outlook, Excel, and the internet.

Working environment/physical activities:

  • Office environment within a clinical setting. Moves about the office environment and to other locations. Moves objects up to 25lbs and 50lbs occasionally. Communicates information to others. Discerns/analyzes information from others to assist in decision making. Enters data into computer programs via computer, mouse, and keyboard.
