Position Summary:

The Substance Abuse Counselor assumes designated responsibility to provide and coordinate substance abuse assessments, treatment planning and service delivery tailored to the individual needs of Members with co-occurring disorders of mental illness and substance use. Also, the Substance Abuse Counselor assumes responsibilities of other functions such as providing treatment, rehabilitation, care management and support services within the team.

Required Knowledge, Skills, Licensure, Training & Travel Requirements (if applicable):

Education:

Must be licensed in North Carolina as an addiction/substance use specialist with at least one-year of full-time experience treating individuals with Co-Occurring disorders and who are dually diagnosed with mental health and substance use disorders.

Qualified Professional (QP) status is required with a designation of CCS, LCAS, LCAS-A, or CDAC.


Work experience
:

A minimum of one (1) year work experience in the human services field as a Substance Use Counselor with the population served.


Knowledge, Skills & Abilities
:

General knowledge regarding principles of Assertive Community Treatment Team (ACTT) to include the laws, regulations, and policies that govern these services. Knowledge of substance use principles, techniques and practices, and their application to complex casework, group work, and community problems. Ability to express ideas clearly and concisely. Possess considerable knowledge of family and group dynamics and a range of intervention techniques. Understanding of Managed Care processes, contracts, and submission of service authorizations. Ability to be available for crisis situations.


Interpersonal Skills
:

Must possess skills in establishing a rapport with members, applying techniques of assessing psychosocial and behavioral aspects of a member’s problems. Ability to establish and maintain effective working relationships with Members being served by the ACT Team. Ability to express ideas clearly and concisely to plan and execute work effectively.

Travel Requirements:

Valid Driver’s License and insurance required. Travel will vary depending on the daily schedule for this position. All travel is in the community where the members live and work.

Provide individual, family, or group counseling to persons with co-occurring substance use and mental health problems in community settings. Continuously develop and implement specialized recovery care services which emphasize community reintegration. Directly assist clients in their daily efforts to build personal recovery skills and habits for self-determination and self-reliance, including use of harm reduction model interventions. Provide ongoing assessment of clients’ symptoms and response to treatment. Make appropriate changes in treatment plans to ensure immediate and appropriate interventions are provided in response to changes in mental status or behavior which put member or others at risk.

Conduct a comprehensive assessment of use of drugs or alcohol and the effects of the addiction on the individual physical, social, and emotional health. Provide service coordination for an assigned group of clients including coordinating and monitoring the activities of the individual treatment team (ITT); assume responsibility for developing, writing, implementing, evaluating, and revising overall treatment goals in collaboration with the client and team.

Organize and facilitate “recovery readiness” and “relapse prevention” groups offering clients new, healthier ways to overcome recovery roadblock and develop self-esteem. Facilitate client referral to self-help groups (AA, NA, etc.) and sponsors who are accepting individuals with a co-occurring mental health diagnosis. Plan, structure, and prompt social and leisure-time activities on evenings, weekends, and holidays to improve social skills of clients. Provide crisis response after hours on a rotating schedule. Conducts comprehensive substance abuse assessments considering the relationship between substance use and mental health. Assesses, tracks and documents individual’s stages of change readiness and stages of treatment. Uses outreach and motivational interviewing techniques to work with individuals in earlier stages of change readiness. Facilitates access to 12-step groups and other community support.

Provide symptom education to enable clients to identify their mental illness or relapse symptoms.
Provide ongoing assessment, problem-solving, side-by-side skill training, supervision (e.g., prompts, assignments, monitoring, encouragement), and environmental adaptations to assist clients with activities of daily living, securing housing, and having adequate financial support.

5.

Facilitates the Person-Centered Planning process for individuals as assigned, serves as a consultant and educator to fellow ACT team staff on the topic of Integrated Dual Disorder Treatment (IDDT) and motivational interviewing.
