What You Will Do as a Social Services Designee

As a Social Service Designee, you will be responsible for the delivery of social services to patients and their families. Some of your primary responsibilities include:

• Handling pre-admission and admission interviews, informing patients and families of available services and patient rights
• Preparing social histories for use by professional center staff and participating in patient care conferences
• Handling problems of adjustment to the long-term care center environment and maintaining frequent contact with patients and their families, serving as their advocate
• Establishing working relationships with community health, welfare, and volunteer groups to promote cooperative assistance in meeting the social welfare needs of patient and families
• Assisting with arrangements, if needed, for out of center health care, transportation, and counseling services
• Handling discharge planning and aiding patients and families on request

Education, Licensure & Certification Requirements

• Master’s degree in Social Work (MSW) OR a bachelor’s degree in Social Work (BSW) OR a concentration in the social sciences
• Minimum of one (1) year of experience in some capacity working with older adults
