We are looking for a full-time MDS coordinator. In this role you will work closely with the DON and Administrator completing the Minimum Data Set (MDS) assessments, Resident's care plan and coordinating patient care with patient insurance companies. The wage range is negotiable and is not set in stone, it's dependent on experience and skills.

• Registered Nurse preferred but LPN will be considered
• Minimum 1-3 years as MDS Coordinator and/or related experience preferred, but willing to train candidates without experience
• Must have excellent interpersonal as well as technical skills
• RAC Certification preferred but not required
• Ability to teach/coach staff on MDS, charting and other topics as needed related to MDS
• Ability to communicate effectively with insurance companies, case managers, and other facility staff members

Pay: $34.00 - $40.00 per hour
