JOB SUMMARY: The PCCA / On call LPN is responsible to, under the direction of the RN PCC, contribute to the physical, emotional, and spiritual needs of the patient and the family/caregiver. He/she contributes to hospice services utilizing the interdisciplinary team approach and provides quality nursing care under the direction of the PCC, attending physician, Medical Director and according to the established Plan of Care. Maintain confidentiality of information relating to patients. Will discuss only those aspects directly involved in the patient's care. QUALIFICATIONS:
Currently licensed as a Licensed Practical Nurse in the State of Oklahoma.
One year's experience in hospice, home health, long-term care, oncology, or geriatric nursing desirable.
Having and maintaining a valid Oklahoma Driver's License and satisfactory driving record is a condition of employment.
PHYSICAL REQUIREMENTS:
Must have strength and stamina for prolonged walking and standing
Demonstrates adequate vision, or correctable with glasses, to read and write.
Able to hear, or correctable with hearing devices, and able to talk to converse with others in person and over the phone.
Able to hear, see, and feel to accurately assess, monitor, and administer care.
Able to frequently life at least 60 lbs., reach, pull, push, bend, stoop, and kneel.
Possess fine motor skills and hand/eye coordination to give care (i.e., vital signs, feed patients, and handle instruments.)
Driving skills and automobile liability insurance required.
