Stillwater Hospice of Montana is currently looking for a full time Social Worker.

Qualifications include but are not limited to:

Masters of Social Work, or LCSW preferred, but will consider BSW or related field of study considered on case by case basis.

Responsible for managing all aspects of the provision of patient social services. Establishes, implements, and evaluates goals and objectives for social services to meet and promote the standards of quality and contribute to the organizations mission and philosophy. Experience in the delivery of health care in home settings, preferably in hospice and or home health. Individual should be able to establish and maintain positive relationships with customers/patients and referral sources and appropriately respond to customer requests and concerns. Individual must meet the training requirements of the State of Montana. Individual should demonstrate good communications skills, autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.

Job responsibilities include but are not limited to:

-Directs all daily patient social service operations including providing direct oversight of the establishment and implementation of social service policies.

-Ensures seamless home care service delivery by providing direct oversight of coordination of care with other clinical service providers.

-Assures proper documentation of social services in clinical records in compliance with state, federal, and CHAP standards.

-Directs the implementation of improved work methods and procedures to ensure social services are delivered in accordance with policy.
