Full-time Home Health Registered Nurse (RN)

Registered nurses (RNs) play a vital role in delivering and coordinating patient care. They are responsible for assessing and monitoring patients, administering treatments and medications, educating individuals and communities about health conditions, and providing invaluable advice and emotional support to patients and their families.

Job Description

As a Home Health nurse, you will deliver personalized care to patients in the comfort of their own homes. This may include wound care, medication management, hands-on patient assistance, administration of intravenous therapy, observing and monitoring patients' conditions, maintaining accurate records, and making physician recommendations.

In this role, you will be responsible for:

• Performing SOC Start of Care assessments
• Conducting evaluations
• Completing follow-up visits
• Managing discharge and re-certification processes
• Being available for 5 days a week, Wednesday through Sunday

Qualifications

• Active and unrestricted nursing license in the State of California
• Minimum of 1 year of professional nursing experience in Home Health or Acute Care settings
• Valid Driver's License with clean driving record (DMV)
