The Role of a Care Coordinator in Waynesboro, GA

Job Description : As a Care Coordinator, your responsibilities will include :

• Effective communication and coordination of care needs reported by case managers
• Updating schedules and ensuring continuity and quality of care for patients
• Facilitating case conferences and following up on action items
• Providing coaching and mentorship to clinicians and CSS role
• Addressing personnel concerns and supporting team development
• Supervising and directing the work of team members
• Managing the site in the absence of higher-level managers
• Overseeing clinical, administrative, and recordkeeping functions
• Responding to phone inquiries in a timely and compassionate manner
• Participating in on-call nursing schedule and providing skilled nursing care in patients' homes

Requirements of Qualified Candidates :

• Must be a registered nurse with current licensure
• Must possess and maintain CPR certification
• Previous home care experience and management or supervisory experience preferred
• Proven ability to work within an interdisciplinary setting
• Must be a licensed driver with a reliable and insured automobile
• Knowledge of professional standards and regulations
