Your new Home Health LVN Responsibilities:

  • You will assist a clinical team covering a commutable caseload territory.
  • A daily caseload supporting 3-5 patients or about 15-17 per week!
  • Quite possibly, you will experience shorter commuting distances with us.

You will bring the following skills as a new Home Health LVN on our team:

  • Make a difference with quality patient care while honing your skills.
  • Prior experience as a field Home Health LVN is preferred, but clinical LVN skills and a desire to learn Home Health will qualify you for a position as our new Home Health LVN!
  • Your stable work history, passion for working in a patient-centered environment, prior post-acute care experience, and any related healthcare experience are all definite pluses to join our cohesive clinical team as our new Home Health LVN.
  • An ASN degree is preferred, providing you with an advantage when qualifying for this Home Health LVN position.
  • Due to commuting distance and caseloads, to be considered for our program, hopefully, you reside in Atherton, East Palo Alto, Los Altos, Menlo Park, Mountain View, Palo Alto, Portola Valley, Stanford, Woodside or Sunnyvale. If you are living in adjacent cities, please do not apply for this position because of the lengthy commute time and distances.
