Step by Step In Home Care LLC is Hiring a Program Coordinator ( DDA Division) Near Great Mills, MD

Yearly : 45K to 50K

JOB PURPOSE: Under the supervision of the Director & Client & Staffing Director the Program Coordinator will have responsibility for carrying out the day-to-day functions of all clients & client's home. This includes securing the well-being and safety of individuals with disabilities which requires; assisting individuals with personal care and daily living skills; training and supervision of Direct Support & Personal Support staff; maintaining house/site facilities; and a broad range of program wide administrative tasks within designated area.

This position requires a commitment to individual choice and community integration and inclusion. Travel by car is necessary and a great deal of time will be spent in individual houses/sites. This position requires the ability to work where needed within the Tri-County area. In addition, responding to emergencies during and after hours, on weekends, and holidays is required when applicable in St.Mary's.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
COMMUNITY LIVING SKILLS & SUPPORTS: ( Only when covering a shift)

  • Assist individual to meet his or her physical (e.g., health, grooming, toileting, eating) and personal management needs (e.g., human development, human sexuality), by teaching skills, providing supports, and building on individual strengths and capabilities.
  • Assist individual with household management (e.g., meal prep, laundry, cleaning, decorating) and with transportation needs to maximize his or her skills, abilities, and independence (or performs the same if the individual(s) cannot do so.
  • Assist with and/or dispense medication (as certified), or other prescribed medical treatments, as necessary.
  • Support individual in the development of friendships and other relationships.

SUPERVISING DIRECT SUPPORT PROFESSIONAL (DSP)

&

PERSONAL SUPPORT STAFF (PSP)

  • Schedule planning and review
  • Being on call via phone
  • Maintaining adequate staffing ratios
  • Approve leave/timecard review and submit to Client & Staff Coordinating Director for review
  • Fill in for staff when needed to cover the shifts
  • Assist the office team with interviewing and hiring of potential direct support & personal support staff
  • Provides individualized orientation to new staff members, mentors and provides appropriate/constructive feedback, when applicable
  • Assures adherence to policies and procedures through training feedback
  • Delegating and assigning duties to DSPs & PSPs
  • Ensuring review/ signatures of all plans (nursing, behavior, etc.)
  • Monitoring electronic health record for case notes and documentation (client dashboard)
  • All assigned tasks are completed daily and logged accurately
  • Complete house inspections/fire drills and monthly attendance
  • Coordinate/facilitate team meetings

FINANCES/MANAGEMENT

  • Ensures that homes operate appropriately and within budgetary guidelines as well as maintains the highest possible quality of living standards
  • Ensuring Grocery list for any homes are turned in
  • Complete the PCP in the electronic health record for review
  • Training staff on all Plans
  • Review all incident reports for accuracy and corrections before submitting for review
  • Submitting all incident reprts into PCIS

HEALTH AND WELLNESS

  • Responsible for all medical appointments/paperwork
  • Primarily responsible for tracking, carrying out and documenting all medical appointments for the individuals, delegating to DSP - PSPs as needed
  • Ensure the activitites manager schedule's and plan activities for consumers to participate in weekly/monthly. Review with activities manage befroe finalized
  • During emergencies, Manager for coverage while assisting at the hospital
  • Review with client & staffing director routine and emergency medical/dental services and medication reviews as appropriate
  • Ensures that DSPs & PSPs monitors (to ensure proper nutritional value) and/or consumer hygiene practices

OTHER DUTIES & RESPONSIBILITIES

  • May perform other duties as assigned

EDUCATION AND EXPERIENCE REQU DIREMENTS ( Minimum )

  • Possesses a High School Diploma or equivalency is required. However, an Associate Degree in human services or related field is preferred.
  • Two years of progressively responsible experience working with people with developmental and intellectual disabilities or a combination of education and experience equivalent to this requirement
  • Must be personable, energetic and have strong communication skills, both orally and in writing.
  • Must have knowledge of community resources available
  • Must have strong interpersonal skills
  • Willingness to work anywhere within the Tri-County area (Calvert, Charles and St. Mary’s counties) as needed. Will be able to use company car during office hours
  • Customer service-oriented
  • Computer skills (familiar with Microsoft Office applications)
  • Has a good command of the English language both oral and written and demonstrates these skills.
  • Possesses a valid driving license with a good driving record, and access to a safe vehicle for transporting people receiving services.
  • Possesses the ability to pass the criminal background check mandated by the State of Maryland.

WORK ENVIRONMENT

  • While performing the duties of this job, the employee is generally not exposed to weather conditions.
  • The noise level in the work environment is usually moderate.
